Participants enrolled in our Supported Living Services can anticipate a comprehensive array of support, training, coordination, and supervision, all strategically designed to nurture self-direction and problem-solving skills across various areas. Our commitment is to address the unique needs and interests outlined in the participant’s PersonCentered Plan (PCP) with a personalized touch
Our Supported Living services are dedicated to helping participants:
a. Cultivate self-direction and problem-solving abilities related to daily living activities and
instrumental activities necessary for independent living.
b. Engage in community-based activities of their choice, within their personal resources.
Divine Help Healthcare’s Supported Living services empower participants to:
a. Reside in a home of their choice, located in their preferred area.
b. Live with chosen individuals or fellow participants (excluding relatives, legal
guardians, or legally responsible persons).
Our services encompass assistance and facilitation in locating suitable housing, roommates, and shared supports, aligned with the participant’s preferences and choices.
We actively oversee quality management and ensure compliance with program requirements once the living arrangement is established.
Divine Help Healthcare Services’ Supported Living services are administered within the participant’s own residence, be it a house or apartment. This includes the provision of coordination, training, support, and supervision, as outlined in the Person-Centered Plan (PCP).
