Health Case Management And Delegation

Our Health case management program focuses on our participant’s well-being in accordance with their healthcare needs. Our nurses manage and assist participants and their family with all their healthcare needs and provide necessary resources. We develop a comprehensive plan of care personalized to meet all physical, functional, psychosocial, environmental, and safety needs of the participant. Our goal is to preserve the participant’s health, ensure a safe environment for them, and improve the quality of life through ongoing monitoring, assessment, and advocacy.
